Tattoo artist license in Florida: how to apply
A Florida tattoo artist license is what permanent makeup legally requires, and it is issued by the Department of Health, not the Board of Cosmetology. ANNAZUR Academy in Jacksonville prepares the application with each student before the hands-on days begin.
The usual order is backwards. People finish a course, feel ready, and then discover that the thing standing between them and a paying client is a form, a county office and an establishment they do not have.
Why this is the bottleneck
Permanent makeup is tattooing in Florida. It is regulated by the Department of Health rather than the Board of Cosmetology, and an esthetician license does not authorize it. So a certificate from any course, ours included, does not let you work.

What does is a tattoo artist license, and the application has a line on it asking which licensed tattoo establishment you work in or intend to work in. That line is where most people stall.
What the assistance actually is
Before the in-person training starts, you and your trainer prepare and complete the necessary paperwork together. Your trainer walks you through the application process rather than handing you a link.
The intention is simple: the application is filed early so the license can be issued in time, instead of you finishing a course and only then discovering what the paperwork involves. Florida requires an active tattoo artist license even for a student tattooing a live model during training, so the license has to be in force — not merely applied for — before that part of the course.
Stuck on the license rather than the technique? That is exactly what this part of the course is for.
The establishment line, solved
Tattooing has to happen in a licensed tattoo establishment, and the establishment holds its own license separately from yours. Both ANNAZUR locations are licensed tattoo establishments, which is the part most applicants spend weeks chasing.
It is also why the sequencing works: the place you train is a place you can legitimately name.
What we still cannot do
We cannot issue a license. No private studio in Florida can, and anyone telling you otherwise is selling something that does not exist. The license comes from the Department of Health and the decision is theirs.

What we can do is make sure your application is complete, correct and submitted in the right county — you apply where you live, not where you plan to work, which catches people out every year.
The requirements, briefly
You must be at least eighteen with government-issued photo identification, and you must complete a Department-approved bloodborne pathogen and communicable disease course and pass its exam. It has to be the Florida version written to the state rule; a certificate from another state or industry will not be accepted.
There is no apprenticeship requirement and no minimum school hours. Rules and county fees change, so confirm current requirements with the Florida Department of Health or your county health department.
Why it matters more than it sounds
Compare the two routes honestly. Skin care means a licensed cosmetology school and a substantial number of classroom hours before you may touch a paying client. Permanent makeup means an approved course, an application and a licensed room.
It is a short road, and the only genuinely difficult step on it is the one this course does with you. Program details are on the course page.
Also in this series:
The short version.
- Tattooing, not cosmetology: an esthetician license does not cover permanent makeup.
- Done in parallel: the paperwork is prepared before the hands-on days, not after.
- The establishment line: both ANNAZUR locations are licensed tattoo establishments.
- County of residence: you apply where you live, not where you will work.
- No license from a course: we help you apply; the Department of Health decides.

Does the course include help with the Florida license?
Yes. You and your trainer prepare and complete the paperwork before the in-person days, with guidance through the application.
Can ANNAZUR issue me a license?
No. No private studio can. The license comes from the Department of Health.
Do I need an esthetician license first?
No. Permanent makeup is regulated as tattooing, which is a separate route entirely.
What is the establishment requirement?
Tattooing must happen in a licensed tattoo establishment, which you name on your application. Both our locations are licensed.
Where do I apply?
In the county where you live, not the county where you intend to work.
Is an apprenticeship required?
No, and there is no minimum number of school hours either.
